PREMIER INFECTIOUS DISEASE
Post Acute Practice Manager Full Time | In Person M-F | 9am - 5 pm | Travel Between Facilities as Needed | $57,000 to $60,000 annually Join a Growing Infectious Disease Practice Premier Infectious Disease is seeking a highly organized, proactive, and relationship focused Post Acute Practice Manager to lead and manage the operational needs of our growing post-acute service line. This position serves as the primary operational point of contact for our post-acute facilities and reports directly to the Director of Operations. The Manager will maintain accountability for provider scheduling, credentialing, education, facility readiness, and the operational support required to successfully deliver infectious disease services across our partner facilities.   • Lead and manage the daily operational needs of Premier Infectious Disease's post-acute service line.
  • Serve as the primary operational leader and point of contact for assigned post-acute facilities.
  • Develop strong working relationships with facility leadership, clinical teams, administrative contacts, and other key partners.
  • Lead new facility onboarding, implementation, and operational readiness.
  • Maintain organized records of facility requirements, contacts, workflows, provider coverage, and operational expectations.
  • Own the resolution of operational issues, escalating significant risks or barriers to the Director of Operations when appropriate.
  • Communicate trends, concerns, opportunities, and recommendations directly to the Director of Operations. Provider Scheduling and Coverage
  • Manage physician and advanced practice provider scheduling and coverage across assigned post-acute facilities.
  • Maintain accurate schedules and communicate coverage to providers and facility partners.
  • Maintain accountability for identifying and resolving coverage gaps before they affect facility operations or patient care.
  • Lead the operational onboarding of providers into new facilities and service locations.
  • Ensure providers have the information, access, and resources needed to successfully provide care at each assigned facility. Credentialing and Facility Onboarding
  • Manage provider credentialing and recredentialing requirements for assigned facilities.
  • Establish credentialing timelines and maintain accountability for the timely completion of all outstanding requirements.
  • Maintain credentialing trackers and proactively monitor outstanding items, expirations, and deadlines.
  • Work directly with providers, facility credentialing teams, and internal resources to obtain required documentation.
  • Maintain accurate provider and facility credentialing records.
  • Communicate credentialing progress, barriers, and anticipated completion dates to leadership. Billing and Provider Education
  • Develop, coordinate, and oversee billing education and workflow expectations for providers serving post-acute facilities.
  • Maintain accountability for ensuring providers understand documentation, billing, and operational requirements specific to each facility.
  • Partner with billing and revenue cycle resources to identify documentation or billing trends requiring provider education.
  • Coordinate follow up education when opportunities for improvement are identified.
  • Monitor and report provider completion of required billing education and operational training.
  • Facility Relationships and Events
  • Manage operational relationships with existing post-acute facility partners and support the development of prospective facility relationships.
  • Coordinate meetings, educational sessions, facility events, provider introductions, and other relationship building activities as needed.
  • Represent Premier Infectious Disease professionally when interacting with facility leadership, providers, patients, and community partners.
  • Lead or coordinate facility meetings, educational programs, relationship events, and assigned organizational initiatives.
  • Travel between post-acute facilities throughout Southwest Florida as needed to support operations, facility relationships, meetings, and events. Operational Excellence
  • Develop and maintain operational trackers, performance reports, schedules, facility information, and service line documentation.
  • Monitor operational priorities and ensure assigned projects are completed timely.
  • Identify opportunities to improve efficiency, communication, provider experience, and facility satisfaction.
  • Lead process improvement initiatives that strengthen efficiency, provider performance, communication, and facility satisfaction.
  • Assist with additional operational projects and responsibilities as assigned by the Director of Operations.
  • Establish and monitor key performance indicators for scheduling, credentialing, provider readiness, billing education, and facility satisfaction.
  • Prepare monthly operational updates for the Director of Operations, including performance trends, risks, barriers, and recommended solutions.
  • Develop standardized workflows and operational procedures for the post-acute service line.
  • Support post-acute growth and expansion by assessing operational readiness for new facilities and providers.
Who We Are Looking For We are looking for someone who takes ownership. The right candidate is organized enough to manage credentialing deadlines, scheduling, facility requirements, provider needs, and multiple priorities without losing sight of the details. They are also personable enough to build strong relationships with physicians, advanced practice providers, facility administrators, clinical teams, and internal staff. Most importantly, they do not simply identify problems. They work toward solutions. Qualifications
  • Three or more years of experience in healthcare administration, physician practice operations, post-acute care, provider scheduling, credentialing, or a related healthcare operations role preferred. Strong organizational and project coordination skills with exceptional attention to detail and follow through.
  • Previous experience independently managing projects, operational workflows, facilities, or provider support responsibilities strongly preferred.
  • Strong verbal and written communication skills.
  • Ability to manage multiple facilities, providers, deadlines, and priorities simultaneously.
  • Ability to work independently and take ownership of assigned responsibilities.
  • Professional and confident when communicating with physicians, facility leadership, employees, and external partners.
  • Proficiency with Microsoft Office, spreadsheets, electronic medical records, scheduling platforms, and other healthcare technology.
  • Experience with provider credentialing, medical billing, physician scheduling, post acute care, skilled nursing facilities, inpatient rehabilitation facilities, or long-term acute care hospitals is highly valued.
  • Associate or bachelor's degree in healthcare administration, business administration, or a related field preferred.
Relevant healthcare experience may substitute for formal education.
