We are seeking an experienced and collaborative Integrated Care at Home Practice Manager to oversee healthcare operations, care coordination, quality improvement, and practice performance while advancing whole-person care. What You'll Do Oversee day-to-day operations of Integrated Care at Home services, including care coordination, workflow management, and service delivery. Supervise and support clinical, administrative, and operational staff. Monitor quality, utilization, operational, and financial performance metrics. Ensure compliance with Medicare, Medicaid, commercial insurance, and other payer requirements. Oversee EMR/EHR systems, care management platforms, reporting tools, and operational dashboards. Develop and maintain relationships with healthcare providers, referral sources, vendors, and community partners. Support quality improvement initiatives, policy development, and operational process improvements. Monitor reimbursement processes, documentation standards, and revenue cycle activities. Identify opportunities to improve efficiency, service delivery, and participant outcomes. Collaborate with leadership on strategic growth initiatives and service line development. Qualifications Bachelor's degree in Healthcare Administration, Business, Public Health, Social Work, Nursing, Human Services, or a related field required. Equivalent combinations of education and experience may be considered. Experience in healthcare operations, care coordination, quality improvement, and healthcare reimbursement systems required. Experience supervising staff and managing healthcare programs preferred. Experience with Medicare, Medicaid, commercial insurance, and value-based care models preferred. Proficiency with EMR/EHR systems, healthcare reporting tools, data analytics, and Microsoft Office applications.
