We are looking for a detail-oriented Medical Records Clerk to support documentation review and provider outreach for a growing healthcare team in Centennial, Colorado. This Contract to Permanent position is ideal for someone who can evaluate clinical records thoughtfully, manage follow-up communication with medical offices, and stay effective in a changing environment. The role requires strong judgment, professionalism, and confidence working across electronic systems while helping ensure records meet established guidelines.

Responsibilities:

Examine patient charts, progress notes, and supporting medical documentation to confirm completeness, accuracy, and alignment with required standards. Process certificates and related records by reviewing details carefully and making informed decisions based on clinical documentation. Place outbound calls to physicians' offices and other healthcare providers to obtain missing records, verify documentation status, and follow up on outstanding items. Respond to incoming calls professionally and assist with questions related to medical documentation and record processing. Navigate multiple electronic systems throughout the day, including newer tools that support document review, and apply independent judgment when validating flagged information. Assess records highlighted by automated review technology and determine whether the documentation supports qualification criteria. Maintain organized documentation workflows and update records consistently to support timely processing. Participate in weekly team meetings to share updates, discuss case progress, and stay aligned on priorities. At least 2 years of relevant experience, with 3 to 4 years in a medical or clinical documentation environment strongly preferred. Hands-on experience reviewing medical records, charts, or related healthcare documentation in an electronic medical record system. Working knowledge of diagnosis codes and other medical coding terminology used in clinical records. Ability to think critically, investigate documentation thoroughly, and make sound decisions based on record content. Detail-oriented communication skills with the ability to build productive relationships with healthcare providers and office staff. Comfort handling outbound provider calls and managing documentation follow-up in a fast-paced setting. Adaptability and willingness to learn new systems, tools, and processes as the role evolves. Experience with durable medical equipment is a plus.
