The Medical Records Specialist is responsible for the accurate, timely, and HIPAA-compliant management of patient medical records within the Homecare Homebase (HCHB) Electronic Medical Record (EMR) system. This role supports clinical and administrative teams by ensuring documentation integrity, physician order compliance, record maintenance, and workflow coordination to facilitate high-quality patient care.
Responsibilities include:
Process and manage medical records, Release of Information (ROI) requests, and Additional Documentation Requests (ADRs) in compliance with HIPAA regulations. Upload, scan, organize, and maintain patient records within the
HCHB EMR
system. Ensure all documentation is complete, accurate, and compliant with regulatory standards. Maintain confidentiality and security of patient information and ensure proper archiving and retention of records in accordance with state and federal regulations. Monitor and track aged unsigned physician orders, submit follow-up requests, and coordinate with physician offices to obtain timely signatures. Track and follow up on unsigned orders, addendums, and other required clinical documentation. Communicate effectively with physicians, clinical teams, marketing staff, and leadership to support documentation compliance. Manage Face-to-Face (F2F) documentation workflows by ensuring encounters are documented, tracked, and completed in a timely and compliant manner. Monitor compliance with Medicare Face-to-Face requirements and collaborate with clinical staff to improve documentation accuracy and workflow efficiency. Partner with nursing, therapy, and social work teams to support coordinated patient care through accurate documentation. Support Clinical Information Systems (CIS) staff and field users with HCHB-related issues. Develop and maintain positive working relationships with physicians, nurse practitioners, physician assistants, and referral sources. Maintain a strong working knowledge of the
HCHB EMR
system, including upgrades, enhancements, and related technologies. Assist with internal and external audits, prepare documentation for reviews, and serve as a liaison for EMR-related issues. Participate in recurring workgroups, special projects, and leadership meetings. Perform other related duties as assigned. Qualifications High school diploma or equivalent required; Bachelor's degree in Health Information Management, Healthcare Administration, or a related field preferred. Minimum one year of healthcare administrative experience required; home health or home care experience preferred. Knowledge of HIPAA regulations, medical record confidentiality requirements, and Medicare Face-to-Face (F2F) documentation standards. Proficiency with electronic medical record (EMR) systems, preferably Homecare Homebase (HCHB), and Microsoft Office applications. Strong organizational, communication, and time-management skills with the ability to manage multiple priorities in a fast-paced environment.
