Job Description

Job Summary The Medical Records Clerk is responsible for organizing, scanning, and maintaining patient medical records. This position ensures records are accurate, complete, and available when needed. Responsibilities Scan and upload patient records into the electronic medical record system. Make sure documents are filed under the correct patient and document type. Request medical records from hospitals, doctors' offices, and other facilities. Follow up on records that have not been received. Process patient requests for medical records. Send records to the appropriate provider or department. Review charts for missing or incomplete records. Process referrals, test results, ABNs, and other medical documents. Handle STAT and urgent record requests. Maintain patient privacy and follow HIPAA guidelines. Keep medical records organized and up to date. Communicate with patients, providers, hospitals, and other medical offices. Assist other departments with medical record needs. Perform other duties as assigned by management. Qualifications High school diploma or equivalent. Previous medical office or medical records experience preferred. Basic computer skills. Good attention to detail. Organized and able to manage multiple tasks. Good communication skills. Ability to work independently and as part of a team. Must maintain patient confidentiality. Work Environment This is a fast-paced medical office position that requires accuracy, organization, and the ability to prioritize daily tasks.
