POSITION SUMMARY
Process incoming and outgoing medical record information accurately and efficiently, following state, federal, and medical records guidelines. Maintains integrity of paper chart storage and retention. Process referrals as ordered and provide all necessary medical records, prior authorizations, and tests as required by specialist appointment. Serve as a resource for all requestors, patients, and staff. This position reports to the Clinical Manager of the site.
PRIMARY ACCOUNTABILITIES
Achieve Results Ensure that all records collected, processed, entered, and filed are done so in a timely, accurate manner. Ensure all clients/patients served experience a high level of customer satisfaction and care. Provide support for additional administrative and support activities within the office and maintain an acceptable level of productivity and consistency with all tasks as assigned. Operational Excellence Ensure and uphold the confidential requirements of all patient records and manage all daily tasks and activities consistent with HIPAA, state and federal laws and regulations, and clinical policies and regulations regarding confidentiality and security. Ensure all information collected, processed, entered, and filed in a timely and accurate manner. Relationships Develop and ensure effective, positive relationships within and among the clinical staff, patients, vendors, contractors, and related resources. Develop and maintain favorable internal relationships, and partnerships with co-workers, including managers, front desk, and clinical staff. Uphold and ensure compliance with and attention to all corporate policies and procedures, as well as the mission and values of the organization.
PRIMARY TASKS & DUTIES
(Per Individual Site tasks may vary)
- Generate schedules, review charts if necessary, and organize preload with nursing staff per site workflow including but not limited to ED summaries, L&D records.
- Correct any errors identified in charts by reprocessing information. Review charts for correct completion of documentation and clinical information tasks.
- Process patient charts and previous EMR documents into Athena per historical scanning guidelines.
- Process all mail received by paper, fax, and Athena. Scans into Athena separating by site/provider.
- Process incoming correspondence and requests for information via scanning, bar coding, and clinical inboxes.
- Consistently follows State/Federal laws and CHC policies regarding release of medical records.
- Coordinate completion of forms, obtain authorizations, and determine the proper mode of release per office and State policies.
- Retrieve medical information requested by site staff and providers
- Retrieve charts for HEDIS, insurance requests, etc.
- Have active access to Tidal Health and CRISP portal.
- Pull charts, preload, retrieve ED and/or L&D, etc. visit summaries.
- Assist patients with referrals including specialists, imaging, and other services as indicated.
- Provide requisitions for all required testing needed before specialist appointment.
- Other duties as assigned.

- Utilize a wide variety of office technology and tools to collect, enter, and process data.
- Utilize Tidal Health and CRISP portal.
COMPLIANCE
This position requires compliance with Chesapeake Health Care Requirements of Conduct/Work Rules, Compliance Program, and policies and procedures. Failure to comply, which may include the failure to report any conduct or event that potentially violates legal or compliance requirements of our written standards, will be met by the enforcement of disciplinary action, up to and including possible termination, in accordance with our compliance program policy and procedures.
POSITION REQUIREMENTS
Education
- High School diploma or equivalent. Must be able to speak read, write and understand English. Professional
- Demonstrated "skilled" business office experience.
- Demonstrated success in communication, customer service, or working with the general public, preferably in a medical care facility.
- Demonstrated success in general computer competence, including basic Word and potential to be trained on specific patient information, billing, and communication software.
- Knowledge of medical terminology preferred.
- Previous experience working in medical records is beneficial. Physical/Environmental
- Ability to work using a computer screen for up to six hours at a time (visual acuity required).
- Must have manual dexterity for use of keyboard. Ability to remain stationary for periods of up to four hours. Ability to communicate via phone, mail, and in person to resolve disputes, solve problems, etc.

Pay:
$16.00 - $23.00 per hour Expected hours: 40.0 per week
