Job Description
Position Summary The HIM Specialist Sr. is responsible for document imaging, medical record analysis, chart reconciliation, data integrity, and record management functions. This role ensures medical records are accurately prepared, scanned, indexed, analyzed, and maintained within the EMR and document management systems while supporting regulatory compliance and patient information requests. Key Responsibilities Prepare, scan, upload, index, and quality-check medical records in electronic document management systems. Analyze patient charts for completeness and manage provider deficiencies in accordance with regulatory and facility guidelines. Support documentation requests, chart reconciliation, record retrieval, and continuity of care requests. Perform data integrity activities, including chart corrections and duplicate medical record resolution. Process birth and death certificates in compliance with state and facility requirements. Respond to internal and external medical record inquiries while maintaining HIPAA compliance. Meet established productivity and quality standards for document imaging and chart analysis functions. Qualifications 2+ years of Health Information Management, Medical Records, or Document Imaging experience. Experience with EMR/EHR and document management systems (VitalChart ChartFlow preferred). Knowledge of HIPAA, medical record regulations, and healthcare documentation standards. Strong attention to detail, organization, and communication skills. Ability to manage multiple priorities in a fast-paced healthcare environment. Preferred RHIT, RHIA, or HIM-related certification. Experience with chart analysis, provider deficiencies, and data integrity functions. Familiarity with Epic, Cerner, Meditech, or similar EMR platforms. Additional Skills & Qualifications Highschool Diploma 2 years of experience in hospital
Health Information Management Experience Preferred:
RHIT or RHIA Certification Healthcare Information Management, HIM, Scanning, Customer ServiceM-F AND WORKING
