Description:
POSITION SUMMARY
The HIMS Clerk is responsible for accurate and timely clerical and filing duties in the HIMS area including inpatient and outpatient records and statistical, and secretarial functions as directed that may include DRG/APC, data entry, Daily Census, and assisting with Performance Improvement projects as directed.
Requirements:
Filing :
1. Knowledge of terminal digit filing system. 2. Accurately files completed charts. 3. Accurately routes incomplete charts. 4. Follows Outguide procedures for checking records in and out. 5. Delivers medical record charts timely and efficiently. 6. Accurately processes all outpatients, ER, loose filing reports into terminal digit filing order and checks for deficiencies. 7. Keeps all filing current. 8. Can accurately assemble discharged patient records into correct chart order. 9. Keeps charts on the shelves in the aisles neat and orderly. Straightens and moves charts as needed. 10. Creates medical record folders for new patients and on 3rd visit for Batch filed patients. 11. Batch files records accurately.
Medical Record Number Control:
1. Assigns new Medical Record numbers as needed. 2. Monitors and corrects computer Master Patient Index.
Medical Record Retention and Storage :
1. Responsible for purging of medical record charts to storage and implementation of retention procedure for appropriate and confidential destruction of records.
Correspondence/Release of Information:
1. Follows Release of Information policy and procedure for processing requests for health information and patient access to health information. 2. Processes requests for health information from patients, healthcare providers, and others. 3. Retrieves and responds to requests from photocopy services.
