Job Description

Position Summary Performs a variety of technical and clerical functions in the Record Processing area in accordance with departmental policies and procedures including Master Patient Index maintenance, KASPER reporting, Quality Checks of documents, and completing multiple functions within the EMR. Responsible for performing and training on all aspects of the document imaging process to include preparation of documents for scanning and indexing the images. Minimum Qualifications Work Experience Previous Health Information experience preferred. Education Two-year associate degree or certificate in a health related field preferred. Certifications/Licensure Registered Health Information Technician (RHIT) or RHIA preferred. Job Specific Performance Standards The duties listed below are a summary of the major essential functions of this position. The position may require other duties, both major and minor, that are not mentioned, and specific functions may change from time to time. Answers incoming phone calls and greets visitors to department within a timely fashion. Determines nature of call/visit and provides appropriate response, guidance. Refers more complex issues appropriately. Takes messages and/or refers visitor/caller to appropriate person. Under moderate supervision compiles physician suspension list for incomplete/delinquent medical records. Processes birth verifications following departmental procedure. Provides general clerical support to include distribution of materials, faxing, and copying. Receives, sorts and distributes departmental mail. Properly reviews indexing of each document for all chart types with the appropriate identifiers and ensures that any poor quality documents within a batch are rescanned and/or moved in order to ensure legibility and retrievability of imaged documents. Facilitates necessary training and assistance to department personnel, physicians, and other end users regarding aspects of document imaging and provides input to development of the document imaging program. Completes analysis process of all patient types and locations under moderate supervision. Processes birth verifications following departmental procedure. Corrects duplicate medical record numbers by maintaining Master Patient Index and running various reports from EMR. Complete KASPER report process and assist providers with Death Registry. Maintains strict confidentiality of patient identities and patient information. Assists on site auditors in the HIM department with locating correct medical record documentation. Assists in completing EMR reports. Assists customers with Patient Portal connection, log in, and edits. Completes chart rounds daily according to departmental policies.
