Job Description

GENERAL SUMMARY
The Medical Records Clerk manages patient records within the office. Request records from other providers. Sends requested records to other providers and hospitals. Maintains HIPAA compliance. Prepares charts for upcoming appointments. No weekends, nights or holidays.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Assures all patient's records and health history forms are completed for the one-day, three-day, 1 week and 2-week list. Sends portal message and creates tasks for PSHH an ROI if health history is incomplete. Makes contact with facility to request records. 2. Pulls paper charts, if applicable, for 1 week ahead of Bone Density appointment; If no paper chart, verifies not chart is found and mark green line for no paper chart. 3. Check task list and Provider Flow throughout the day to verify old task have been completed. Assure task are completed in a timely manner. 4. Prints referrals, prescriptions and prior authorizations off of Provider Flow. Place prescription/prior authorizations in the appropriate Triage Nurse bins. 5. Verify referral patients are in system. If not in system, take records to Appointments to be entered into system. If in system, place referral in Phone Triage nurse's bin. 6. Places outside physician's progress notes/labs/radiology information in patient's chart. Assures that information is sent to physician for review, if needed. 7. Prints and scans requested patient records from outside physicians to system. 8. Retrieves and scans Emergency room visit information such as visit date, lab results, radiology results, physician reports and discharge instructions through EPIC or fax requests. 9. Scans Tubal Consents and stamps to indicate that document was entered into the system. Forwards all documents to Insurance department. 10. Scans Established New Patients' charts before appointment and places in face sheeting bin in the Med Room. Verifies chart is correct patient. 11. Verifies that new patient has not been seen in office before and that chart is not available. 12. Assures that Face sheeting goes into Face sheeting bin. 13. Completes Record Releases and documents in chart after records are faxed/mailed to indicate what patient information was sent, and where the records were sent. 14. Pulls old charts from file room and scans into Smart Search 15. Shreds old charts once scanned into Smart Search.

EDUCATION/EXPERIENCE
1. High School Diploma or GED certificate 2. Medical office experience 3. Computer knowledge and experience required 4. Excellent oral and written communication 5. Ability to multi-task
