Job Description

Temporary, Short-Term Project Job Description:
A boutique private surgical practice in San Jose is seeking a highly organized, detail-oriented, and reliable individual for a short-term project to clear a backlog of physical office paperwork. The primary focus of this project is to organize, scan, and accurately upload physical documents directly into patient digital charts within our electronic medical record (EMR) system. This is an immediate, temporary position expected to last approximately 1 to 2 weeks, depending on the pace of completion.
Core Responsibilities:
Sort and prepare physical medical records, administrative forms, and patient paperwork for scanning. Operate office scanning equipment to digitize files with high clarity and correct orientation. Accurately index and upload digital files into the correct patient charts inside our EMR system, ensuring strict quality control. Maintain total confidentiality of all patient data and follow strict office data-entry protocols. Securely organize or prepare scanned paperwork for compliant disposal/shredding.
Qualifications & Requirements:
EMR Exposure:
Prior experience working with Electronic Medical Record software or high-volume digital data entry is highly preferred.
Technical Literacy:
Must be highly proficient with computers, desktop scanning software, and filing structures.
Confidentiality & Compliance:
Must have a foundational understanding of patient privacy. The selected candidate will be required to sign a strict Workforce Confidentiality Agreement and complete a brief, practice-provided HIPAA security training module on Day 1. Attention to
Detail:
Absolute accuracy in matching patient names, dates of birth, and document types during the charting process is a strict requirement. Reliable transportation and punctual attendance.
