Full Job Description Summary of Duties:
Responsible for assisting residents with medication administration and with routine daily care and services in accordance with the resident's Medication Administration Record (MAR), resident's assessment and plan of care as directed.
Essential Job Functions:
1.Assist the resident with mediation. 2.Verify with the resident, the resident can recognize the name, date and medication they are to take 3.Verify the resident has taken the medication 4.Provide care to residents including activities of daily living as needed 5.Notify the manager/supervisor any change in condition (i.e. discrepancy in the order, MAR or medication pack, resident's inability to recognize medications, areas of redness, skin tears, bruising, mental status changes, eating habits 6.Compare the resident's physician's order and the MAR 7.Verify the order matches the MAR 8.Verify the order, MAR, and the medication pack match 9.Document appropriately on the
MAR/ EMAR
10.Assist with lifting, moving, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, etc., as necessary, utilizing proper facility technique 11.Check residents routinely to ensure that their personal care needs are being met 12.Assist in preparing and serving meals as necessary and on a timely basis 13.Keep resident rooms and resident care areas clean and neat 14.Perform day-to-day housekeeping activities in accordance with daily work assignments 15.Orient new residents to facility 16.Encourage and assist residents to participate in activities. Assist in conducting activities with residents as directed 17.Assume additional responsibilities as assigned or needed.
Education:
High School diploma or equivalent preferred
Experience:
Experience in resident/patient care preferred
