Medication Management Preparation:
Accurately preparing routine oral medications for residents according to physician orders.
Administration:
Delivering medications to residents in a timely manner, following the "six rights" of medication administration (right resident, drug, dose, route, time, and documentation).
Pharmacy Liaison:
Managing medication inventory by proactive communication with pharmacies to ensure timely refills and availability of prescriptions.
Documentation & Compliance EMAR Recording:
Utilizing Electronic Medication Administration Records (EMAR) to log every dose given, ensuring real-time accuracy and legal compliance.
Clinical Charting:
Maintaining detailed resident charts to track health trends, medication efficacy, or any changes in daily status.
Observation & Communication Clinical Reporting:
Acting as the "eyes and ears" for the care team by identifying and reporting unusual symptoms or physical changes, such as new wounds, skin breakdowns, or behavioral shifts.
Supervisory Updates:
