In this vital role, you will serve as a central point of contact, coordinating patient care activities, managing medical documentation, and ensuring seamless communication across departments. Your energetic approach will help foster a positive environment for both patients and staff, supporting high-quality care delivery in diverse clinical settings. The ideal candidate will bring a passion for patient-centered service. Duties Administer medications only as authorized and within the QMAP's permitted scope. Follow current medication orders and facility medication policies. Verify the right resident, right medication, right dose, right route, right time, and appropriate documentation . Review medication labels and medication records before administration. Document medication administration accurately and promptly. Document medication refusals, missed doses, medication errors, and other medication-related concerns according to facility policy. Notify the appropriate supervisor of medication concerns or errors immediately. Follow procedures for medications that are unavailable, discontinued, refused, held, or otherwise unable to be administered as ordered. Maintain medication security and follow facility procedures for medication storage and handling. Follow procedures for controlled medications, when applicable. Obtain clarification from the appropriate supervisor/provider when a medication order is unclear or cannot be safely followed. Complete required medication-related documentation. Skills Proven experience working with electronic health record (EHR) systems such as Epic, Cerner, or eClinicalWorks. Strong knowledge of medical terminology across various specialties including geriatrics, critical care, emergency medicine, hospice care, and long-term care facilities. Familiarity with HIPAA regulations and maintaining strict patient confidentiality at all times. Experience in assisted living / caregiving. Ability to perform basic life support (BLS), CPR, and aseptic techniques confidently. Excellent caregiver communication skills to effectively collaborate with patients of all ages. Knowledge of medical documentation standards including vital signs recording, medication administration logs, and medical records management.
