Completion of medication histories requires direct patient conversation and accurate documentation in the medical record. This includes collaboration with the patient care team, interviewing patients and/or caretakers and contacting outside healthcare entities (e.g., hospitals, provider offices, pharmacies, etc.) to obtain patient information. The medication history senior technician performs these duties under the general supervision of the clinical pharmacist and pharmacy supervisor. The first-shift schedule for this position is alternating weeks of Tuesday through Friday and Tuesday through Sunday.
Duties to include:
Triage patients for medication history collection based on patient and medication list complexity
  • Utilize tools within the electronic medical record to identify patients appropriate for completion of medication histories
  • Coordinate with pharmacist staff Collect pertinent information and details to complete an admission medication history for reconciliation by the pharmacist
  • Possess strong communication skills for patient and/or caregiver interviews
  • Have a willingness to contact other healthcare professionals at provider offices, pharmacies, and institutions to collect medication history data
  • Concisely but thoroughly document relevant patient allergy and medication lists within the electronic medical record
  • Focus attention to high-risk medications (i.e., anticoagulants, chemotherapy, home parenteral medications, immunosuppressants, etc.)
  • Note timing of last medication doses when relevant. Independently author medication history chart notes within the electronic medical record. Clearly communicate with pharmacist colleagues regarding the status of medication histories either via verbal, written, or electronic means. Coordinate daily technical functions for assigned work areas
  • Provide supervision to pharmacy student interns as requested
  • Reassign work areas and duties for pharmacy student interns based on workload
  • Provide feedback to supervisor regarding performance of colleagues as requested Act as a positive role model
  • Communicate with clear, positive verbal and written messages to individuals within the department as well as with patients, the public, and other departments
  • Foster teamwork by encouraging and assisting others Assist in the training of new colleagues as requested
Minimum Eligibility Requirements:
Registered with the Iowa Board of Pharmacy at the time of hire. National certification through the Pharmacy Technician Certification Board (PTCB) or the National Healthcareer Association (NHA). Must meet ongoing education requirements and maintain certification. High school diploma or the equivalent experience Experience Any combination of four years of post-high school experience and/or education in the natural sciences OR Four years of experience as a pharmacy technician
Desired Qualifications:
