We are seeking an LPN to provide home health nursing services to a client in the Buzzards Bay/Bourne, MA area . The primary responsibility of this position is to assist the client with medication management and medication adherence through the proper use of an electronic medication dispenser. The LPN will work directly with the client in their home to ensure medications are organized, the medication dispenser is functioning properly, and the client is safely receiving medications according to the established care plan and physician/provider orders. Key Responsibilities Provide skilled nursing services in the client's home. Assist with medication management and medication reconciliation as permitted within the LPN's scope of practice. Set up, monitor, and assist the client with the use of the electronic medication dispenser. Verify that medications are properly organized and available according to the established medication schedule. Monitor medication adherence and identify missed, delayed, or refused doses. Assess the client for potential medication-related concerns and report significant changes to the appropriate healthcare provider. Communicate with the client's physician, pharmacy, family/caregiver, and care team as appropriate. Monitor the client's general health status and report changes in condition. Provide education to the client and/or caregiver regarding medication safety and proper use of the medication dispenser. Document all nursing visits, medication-related observations, interventions, and communications accurately. Follow the client's individualized care plan and all applicable home health, nursing, and agency policies. Maintain confidentiality and comply with HIPAA and applicable Massachusetts regulations. Qualifications Current and unrestricted Massachusetts LPN license . Current CPR/BLS certification preferred. Previous home health, private duty nursing, geriatric, medication management, or community nursing experience preferred. Ability to work independently in a client's home. Strong communication, organization, and documentation skills. Reliable transportation and ability to travel to the Buzzards Bay/Bourne, MA area. Ability to interact professionally and compassionately with clients and families. Schedule The schedule can be discussed based on the client's needs and the RN's availability.
