This energetic and rewarding position involves providing personalized care, supporting daily activities, and ensuring safety and comfort for clients with diverse needs. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by checking vital signs and observing changes in condition; report findings accurately Prepare nutritious meals tailored to individual dietary needs and preferences, ensuring proper food safety standards Administer medications following prescribed schedules and maintain detailed medication administration records in compliance with HIPAA regulations Utilize specialized equipment such as Hoyer lifts or wheelchairs to safely transfer clients with mobility challenges Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces Support clients with cognitive impairments such as dementia or Alzheimer's by implementing memory care techniques and engaging activities Qualifications CNA or HHA certification required CMT certification required Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred Knowledge of patient observation, monitoring vital signs, and recognizing emergency situations; CPR and First Aid certifications are required Experience working with individuals with developmental disabilities, autism, or dementia is a strong plus Ability to perform heavy lifting safely and use assistive devices like Hoyer lifts effectively Familiarity with care plans, medication administration, HIPAA compliance, and documentation procedures Compassionate attitude with excellent communication skills; patience and adaptability are essential Valid driver's license and reliable transportation preferred for travel between client homes.
