JOB DESCRIPTION

Position Overview:

As a Care Center Coordinator, you will be responsible for ensuring the well-being, safety, and proper medication management of our residents. This role requires meticulous attention to detail, strong communication skills, and a compassionate approach to resident care. You will oversee medication administration, provide personal care assistance, monitor residents' health status, and handle various administrative tasks to ensure compliance with regulatory requirements and organizational policies.

Key Responsibilities:

Medication Management:

Assist residents with administration, supervision, and monitoring of medications in accordance with Health Facility Administrator (HFA) licensing requirements. Follow the 6 rights of medication management and maintain basic knowledge of medications and adverse reactions, especially in the senior population. Accept new medication orders or changes, ensuring accurate documentation and compliance with licensing requirements. Ensure proper destruction of medications no longer needed, including narcotics, following organizational policies and regulations.

Supply Management and Documentation:

Order medications and supplies as needed using the electronic Medication Administration Record (MAR) system (ECP). Conduct monthly medication cart audits to ensure accuracy and compliance. Document medication administration, resident observations, and incidents accurately and confidently in resident charts. Resident Care and Observation Be available and on call to shift supervisor(s) Assist residents with personal care needs such as bathing, dressing, and ambulating as required. Apply universal infection control measures to prevent the spread of infection during all care activities. Observe residents for changes in physical and mental health status and communicate any changes to the Registered Nurse (RN)/Licensed Practical Nurse (LPN) or Director of Resident Care.

Incident Reporting and Emergency Response:

Complete incident reports for any resident incidents and notify the Director of Resident Care promptly. Notify physicians of changes in resident status or clarification of orders in the absence of the Director of Resident Care. Responsible for calling Emergency Medical Services (EMS) and sending residents to the Emergency Room (ER) if necessary.

Communication and Collaboration:

Serve as a contact for family members or responsible parties regarding questions or concerns in the absence of the Director of Resident Care. Communicate effectively with teammates and the next shift to ensure continuity of care. Attend required in-service meetings and staff meetings, and may be responsible for training new Medication Technicians.

Supervisory and Miscellaneous Tasks:

Serve as the supervisor on your shift in the absence of the Director of Resident Care. Ensure residents leaving the building have their medications and instructions. Accept Advanced Health Pharmacy medication delivery according to proper protocols. Perform other general tasks as observed or delegated by the Director of Resident Care.

Qualifications:

Certification as a Medication Technician or equivalent training. Previous experience in medication administration and senior care preferred. Strong organizational and documentation skills. Ability to communicate effectively with residents, families, and healthcare professionals. Knowledge of regulatory requirements and compliance standards in healthcare settings. Flexibility to work overtime, weekends, and be available in case of emergencies or natural disasters. Every effort has been made to keep this job description as complete as possible.
