Part Time Pharmacy Job Description Job Summary :
To perform medication history assessments for new admissions to the hospital and provide complete information regarding each medication identified (i.e. drug, dose, route, frequency, indication for PRN, and date/time of last dose taken). Medications to be reviewed include prescription and over the counter medications (including herbals, vitamins and dietary supplements). Contact the patient's community pharmacy, physician's office, or other healthcare facility for clarification if needed. Verify patient allergy information and ensure complete documentation (i.e. substance, allergy category, description of reaction). Coordinate medication history assessment with pharmacy, nursing and physician team. Document the medication history and allergy information in the electronic medical record accurately and in a timely manner. Promptly communicate any medication history issues with supervising pharmacist and help to identify and facilitate correction of medication discrepancies
Qualifications :
Completion of 12th grade (high school or equiv); Six month prior pharmacy work experience preferred; Some ability to perform accurate calculations and reports. Some typing skills. Must possess attention to detail and ability to prioritize work in high volume work environment with frequent interruptions. Annual Registration with the NC State Board of Pharmacy is required. A new Pharmacy Technician hired but not already registered with the NC Board has 30 days from hire date to comply. Active Pharmacy Technician Certification (CPhT) is encouraged.
