SUMMARY OF POSITION
Responsible for working with newly admitted patients to obtain and document an accurate medication list to help avoid medication duplications, omissions, and errors. The information obtained will be entered into the electronic health record to be used by physicians and other health care providers.
MINIMUM QUALIFICATION
(S): Current Pharmacy Technician Certification required (PTCB, NHA, or equivalent) Registration as a Pharmacy Technician by the Pennsylvania State Board of Pharmacy required PTCB Medication History Certification required within twelve (12) months of start date Minimum five (5) years of pharmacy technician experience required
