Job Description

Summary As part of the multidisciplinary healthcare team, the Pharmacy Technician Medication History serves as an integral part of the progressive pharmacy managed medication reconciliation process. The Pharmacy Technician Medication History is responsible for collecting a complete and accurate medication and allergy history for patients in the emergency department and on the inpatient units. This information is obtained through interviewing patients and caregivers, and by reviewing records from nursing facilities and retail pharmacies, as applicable. The updated medication list is entered in the patient's medical record and is used by prescribers to determine treatment plans and medication orders during admission and discharge. Minimum Qualifications 1. High School graduate or equivalent. 2. Fundamental understanding of personal computers, keyboarding skills, and familiarity with the Windows operating system. 3. Must meet one of the following: 3 years of experience as a pharmacy technician in a hospital 2 years of experience as a pharmacy technician in a retail setting 1 year of experience as a medication history pharmacy technician. 4. Current certification as Pharmacy Technician or attainment of certification within 1 year of hire. 5. Excellent verbal communication skills. 6. Ability to triage workload independently and appropriately throughout scheduled shift. 7. Ability to solve problems independently and handle challenging patient and family situations commonly seen in acute care settings.
