Role Overview:

The pharmacy technician is responsible for compiling comprehensive medication histories for patients prior to hospital admission by interviewing patients and families, verifying medication details, allergies, and sources of information. They ensure accurate documentation of medication names, dosages, routes, indications, and last taken times, including prescription, non-prescription, and investigational drugs. The technician verifies medication adherence, communicates high-risk medications to pharmacists, and collaborates with healthcare staff to resolve discrepancies.

Key Responsibilities:

Interview patients and families to gather medication information Verify allergies and medication accuracy using available resources Document medication details, including PRN reasons and special instructions Communicate complex or high-risk medication concerns to pharmacists Assist pharmacists with drug preparation, inventory, and record maintenance Ensure confidentiality, safety, and professionalism in all tasks Collaborate with healthcare team members to confirm medication histories Utilize electronic databases and other sources to verify records Perform general pharmacy technician duties and participate in departmental meetings

Qualifications & Skills:

High school diploma or equivalent At least 2 years of retail pharmacy experience; hospital pharmacy experience preferred Registered pharmacy technician in the state; certification preferred ACPE IV certification (including hazardous materials); optional

BLS, ACLS, PALS

Knowledge of medications, dosages, and administration Strong communication skills to interact with diverse patients and staff Ability to organize, prioritize, and maintain confidentiality
