Hourly Pay Range:

$21.34 - $31.48 What You'll Do As a member of our Assertive Community Treatment (ACT) Team , you will work closely with behavioral health professionals in a collaborative, multidisciplinary environment to help members achieve their personal goals and maximize independent living.

Key responsibilities include:

Provide individualized independent living skills training in community and home-based settings. Teach and reinforce skills related to: Personal care and self-management Household management and daily living activities Budgeting and financial literacy Transportation access and community navigation Social and communication skills Building healthy relationships and support systems Recreation and community engagement Problem-solving and decision-making Avoiding exploitation and promoting personal safety Collaborate with ACT team members to develop and implement Individual Service Plans (ISPs). Connect members and families with community resources and support services. Provide recovery-focused, trauma-informed services that promote dignity, empowerment, and independence. Participate in weekend, evening, and on-call rotations as needed. Transport and accompany members throughout the community when appropriate.

Qualifications Education:

Candidates must meet one of the following requirements: Bachelor's degree in a Behavioral Health-related field; OR Bachelor's degree in any field and one (1) year of full-time behavioral health experience; OR Associate degree and two (2) years of full-time behavioral health experience; OR Eighteen (18) college credit hours in a Behavioral Health-related field and two (2) years of full-time behavioral health experience; OR High school diploma or GED and four (4) years of full-time behavioral health experience.

Experience:

Minimum one (1) year of experience working with adults experiencing serious mental illness. Experience working in an ACT team or providing independent living skills training is preferred. Must meet Arizona Administrative Code requirements for behavioral health personnel.

Licensure & Certifications:

Valid Arizona driver's license in good standing. Must maintain Arizona-required automobile insurance coverage. Must meet Valleywise Health Vehicle Use Policy (VUP) driving eligibility requirements.
