Duties Conduct thorough patient assessments to identify medical, behavioral health, and social needs of students. Develop individualized care plans that incorporate medical treatment, mental health support, and social services. Collaborate with healthcare providers, mental health professionals, and community organizations to coordinate services and resources. Manage electronic health records (EHR) accurately while ensuring compliance with HIPAA regulations to protect student privacy. Facilitate discharge planning and follow-up care for students with complex medical or behavioral conditions. Provide client communication and education to families regarding health management, medication administration, and available community resources. Assist in intake processes for new students requiring additional support services or specialized care. Qualifications Prior experience working in hospital settings, Level I or Level II trauma centers, or acute care environments is highly desirable. Demonstrated knowledge of hospital medicine, emergency medicine, ICU (Intensive Care Unit), PICU (Pediatric Intensive Care Unit), and post-acute care experience. Strong understanding of medical terminology, physiology, diagnostic evaluation, and clinical case management practices. Experience working with children and individuals with disabilities or mental health conditions is essential. Proficiency in EMR/EHR systems, HIPAA compliance, and electronic health records management is required. Excellent patient assessment skills combined with task prioritization abilities to manage multiple cases efficiently. Knowledge of community resource coordination in health services social work and managed care practices. Ability to communicate effectively with families from diverse cultural backgrounds while providing compassionate patient service. Relevant certifications such as Basic Life Support (BLS), CPR, addiction counseling credentials, mental health counseling training, or social work licensure are preferred but not mandatory.
