PRSC (Psychiatric Rehabilitation Services Coordinator) All American Village Nursing and Rehabilitation Center Chicago, IL Job Details Full-time 11 hours ago Benefits Paid holidays Paid time off Vision insurance Qualifications Clinical case management notes High school diploma or GED Care plan implementation Therapy progress note documentation Full Job Description All American Village is seeking a compassionate, dependable Psych Rehab Services Coordinator (PRSC) to join our behavioral health team. The PRSC plays a key role in supporting residents with serious mental illness or psychosocial challenges by assisting with the delivery of psychosocial rehabilitation services. This role helps residents achieve greater independence and improve their quality of life through structured support, goal-setting, and participation in therapeutic programming. There is a $4000 sign-on bonus for this position.
Key Responsibilities:
Assist in delivering psychiatric rehabilitation services as outlined in individualized service plans Support residents in developing life skills, social skills, coping mechanisms, and wellness routines Facilitate or co-facilitate group and one-on-one psychoeducational and recreational activities Provide behavioral cues and redirection as needed in alignment with individual behavior plans Document resident participation and progress in accordance with state and facility standards Collaborate with the interdisciplinary team to monitor resident needs and communicate concerns Maintain a safe, supportive, and respectful environment that encourages participation and growth Transport or accompany residents to community outings or appointments when applicable Adhere to all HIPAA, safety, and infection control policies
Qualifications:
Required:
High school diploma or GED Prior experience working with individuals with mental illness, developmental disabilities, or in a behavioral health or long-term care setting Strong interpersonal and communication skills Patience, empathy, and the ability to work in a structured therapeutic environment Ability to document clearly and follow care plans Must pass background check and meet all facility and state employment requirements
Preferred:
Associate's degree or coursework in psychology, social work, or a related field Completion of a certified Psychosocial Rehabilitation Aide training (if required by your state) CPI, First Aid/CPR certification
