Elevated Care Solutions LLC is currently hiring a Mental Health Practitioner (MHP) to provide Adult Rehabilitative Mental Health Services (ARMHS) to adults living in the community. Our agency focuses on helping individuals improve independence, stability, and daily functioning through person-centered mental health services. This role involves working directly with clients in their homes and community settings to support their mental health rehabilitation goals. The practitioner will help clients build skills related to psychiatric stability, independent living, social skills, medication education, and accessing community resources.

Responsibilities:

Provide ARMHS services according to Minnesota DHS guidelines Work one-on-one with clients in community settings Help clients develop and work toward their rehabilitation goals Assist with daily living skills, coping skills, and mental health education Complete documentation accurately and on time Maintain professional and ethical standards at all times Communicate with supervisors and treatment teams as needed Qualifications (Must meet Minnesota DHS Mental Health Practitioner requirements): Applicant must meet one of the following: Bachelor's degree in psychology, social work, or related human services field OR Associate degree in behavioral science field with required experience

hours of supervised experience providing mental health services Applicant must also have: Valid driver's license and reliable transportation Ability to work independently Strong communication skills Ability to complete documentation electronically
