We take a collaborative, multidisciplinary approach, embedding our services directly within the nursing home setting to work alongside facility medical staff and ensure optimal outcomes for residents. Through a blend of group therapy, clinical treatment, and specialized on-site support, we promote long-term rehabilitative and emotional health on the journey to recovery. Position Overview We are seeking an experienced Chemical Dependency Counselor Assistant (CDCA) to deliver integrated SUD and TBS (Therapeutic Behavioral Services) programming directly within our partner skilled nursing homes. In this role, you will provide a comprehensive range of clinical and behavioral support services tailored to nursing home residents facing co-occurring medical, substance use, and mental health challenges.

Credentialing Pathway:

Candidates must hold an active CDCA license upon hire. As a condition of employment, you will also be expected to obtain your Qualified Behavioral Health Specialist (QBHS) certification, which our organization will process and issue for you upon hire.

Core Responsibilities & On-Site Expectations In-Facility SUD Programming:

Facilitate SUD individual counseling sessions, lead SUD outpatient (OP) groups, and develop individualized treatment plans tailored to nursing home residents.

Mental Health & TBS Services:

Deliver Therapeutic Behavioral Services (TBS) individual sessions and co-facilitate Mental Health Day Treatment groups alongside a licensed clinician within the facility.

Assessments & Level of Care:

Perform bio-psychosocial assessments and complete ASAM (American Society of Addiction Medicine) level-of-care evaluations under clinical supervision.

Interdisciplinary Care Coordination:

Collaborate closely with nursing home administrative and clinical staff, attend interdisciplinary treatment team meetings, and coordinate safe discharge or transition plans with community partners.

Documentation & Telehealth:

Maintain timely, compliant EMR documentation adhering to facility, state, and regulatory billing standards. Facilitate telehealth visits and submit required daily reports.

Regional Coverage:

Float across designated skilled nursing home locations in the Columbus Region as needed.

Qualifications & Requirements Licensure:

Active Ohio CDCA (Chemical Dependency Counselor Assistant) license required; prior experience in addiction services or long-term care settings preferred.

Certification Expectation:

Willingness to receive and maintain QBHS (Qualified Behavioral Health Specialist) credentialing, issued directly by Stepping Stone upon hire.

Billing Identifiers:

Active Ohio Medicaid Billing Number and NPI (National Provider Identifier) Number (or eligibility to finalize prior to start).

General Requirements:

Must be 21 years of age or older with a valid driver's license and reliable transportation.

Skills:

Strong organizational skills, attention to detail, strict adherence to confidentiality (HIPAA / 42 CFR Part 2), and proficiency with EMR systems.
