Specialist provides community and office based case management for individuals aged 15-40 who are experiencing a first episode of psychosis or severe mental illness. You will deliver direct support, crisis intervention, and life skills training to help clients function effectively at work, school, and home. Key Responsibilities •
Direct Care & Support:
Provide crisis intervention, daily living skill development (money management, medication routine, social skills), and emotional/behavioral management. •
Care Coordination:
Partner with families, medical staff, and social service providers to execute treatment plans. •
Client Transportation:
Transport clients in your personal vehicle to various community locations, appointments, and daily tasks. •
Documentation & Metrics:
Complete clinical notes within 24 hours, maintain client outcome tracking, and meet agency productivity standards. Role RequirementsEducation & Experience •
Preferred:
Bachelor's degree in Psychology, Social Work, Sociology, or a related field + 1 year of mental health experience. •
Alternative:
High School Diploma + 3 years of full-time experience working directly with severe mental illness. Essential Qualifications •
Driver's License:
Valid Ohio driver's license, clean record, proof of auto insurance and access to a reliable personal vehicle to transport clients. •
Skills:
Basic computer proficiency (MS Office/Windows); experience with Electronic Health Records (EHR) and Spanish fluency are a plus. •
Physical/Environment:
Ability to travel, navigate various home/community conditions (stairs, weather, rough terrain), sit for extended screen time, and handle emergency situations safely. Total Rewards & Perks •
