Position is Full time 40 hours Friday-Monday 8am-6pm

Responsibilities:

Provide individualized, person-centered support to the assigned resident based on their RSP, Interaction guidelines, crisis plan, medical needs, and identified goals. Promote choice, dignity, independence, and skill development while providing the level of supervision identified in the individual's service plan. Utilize positive behavior supports, trauma-informed care, and de-escalation techniques. Observe for changes in physical health, mental health, mood, behavior, or functioning and promptly communicate concerns to management. Implement safety plans, crisis interventions, and individualized support strategies as directed. Encourage community participation, social interaction, recreation, and meaningful daily activities. Although assigned as a 1:1 Support Specialist, employees remain part of the overall staffing team and are expected to: Respond appropriately to incidents, emergencies, and resident needs throughout the home while maintaining the safety and supervision of the assigned resident. Assist coworkers during behavioral or medical emergencies. Support other residents with routine needs when the assigned resident is safe and appropriate supervision can be maintained. Participate in shift responsibilities that ensure the smooth operation of the home. Participating in shift-to-shift communication. Reviewing daily logs, communication books, medical notes, progress notes, and incident reports. Reviewing schedule changes, appointments, staffing updates, and resident needs. Communicating significant observations and concerns to incoming staff and supervisors. Administer medications and treatments as delegated and authorized. Accurately document medication administration and treatments. Monitor and document controlled medications. Observe for changes in residents' health conditions and report concerns immediately. Monitor vital signs when ordered. Provide first aid and obtain emergency medical assistance when necessary. Attend medical appointments with residents as assigned. Ensure medical orders, medication changes, treatment changes, and follow-up recommendations are communicated and implemented. Update documentation and MARs as required. Teaching daily living skills identified within the resident's service plan. Providing both scheduled and naturally occurring teaching opportunities. Encouraging independence while providing only the assistance necessary. Documenting skill development and resident progress. Daily progress notes for the assigned resident. Required RSP tracking documentation. Medical notes. Appointment documentation. Incident reports. Communication logs. Other required documentation before the end of each shift. Prepare meals and snacks following physician orders and dietary requirements. Encourage resident participation in meal planning, grocery shopping, food preparation, cooking, and kitchen clean-up. Monitor portion sizes and dietary modifications. Properly store food according to policy. Bathing as needed Grooming as needed Oral hygiene as needed Dressing as needed Toileting as needed Incontinence care as needed Shaving as needed Encourage participation in recreational, vocational, educational, and social activities. Transport residents using authorized agency vehicles. Promote safe and appropriate community interactions. Assisting with meal preparation and kitchen clean-up. Completing assigned daily, weekly, and routine household cleaning tasks. Maintaining sanitary common areas. Encouraging resident participation in household chores. Reporting maintenance and safety concerns promptly. Assisting with laundry and household organization as needed. Protect resident rights, privacy, confidentiality, and dignity. Maintain a safe living environment. Follow abuse reporting requirements. Report hazards immediately. Follow all infection control procedures. Adhere to emergency procedures and crisis response plans. Minimum Qualifications Ability to read, write, and communicate effectively in English. Ability to document objectively and accurately. Ability to safely supervise residents while completing household responsibilities. Ability to work independently and collaboratively as part of a team. Ability to respond appropriately in emergency and crisis situations. Ability to maintain confidentiality and professional boundaries. Must meet all NW Premier employment requirements, including required certifications and training. The 1:1 Support Specialist's primary responsibility is providing individualized support to the assigned resident; however, this position is not exclusively a one-person assignment . Staff are expected to balance individualized supports with the operational needs of the home, working collaboratively with coworkers to maintain a safe, clean, therapeutic, and person-centered environment for all residents while ensuring the assigned resident continues to receive the level of supervision and support required by their service plan.

Qualifications:

• Strong leadership skills encouraged
• Previous experience in mental health or related field preferred
• Knowledge of hospice care, senior care, dementia care, or behavior management systems is a plus
• Ability to perform heavy lifting and physical tasks as required
• Strong communication and interpersonal skills
• Ability to work effectively in a team setting
• Compassionate and patient-centered approach to care
• Familiarity with ADLs (Activities of Daily Living)
• Experience working in a nursing home or similar setting is beneficial
• Prior medication administration experience
