Overview Join Project Family's dynamic behavioral healthcare team as a Community Support Specialist (Behavioral Health), where you will play a vital role in providing compassionate, comprehensive support to individuals experiencing behavioral health challenges. This position offers an exciting opportunity to work within a multidisciplinary environment, coordinating care, developing skills, and connecting clients with essential community resources. This position provides direct support to individuals facing mental health challenges by helping them navigate resources and develop coping skills. A Community Support Specialist is a member of a therapeutic team that assists clients with their treatment plans and mental health goals.
Responsibilities Support and Guidance:
Assist clients in understanding and managing their mental health conditions.
Treatment Planning:
Collaborate with interdisciplinary teams to develop and implement treatment plans.
Resource Navigation:
Help clients access community resources and services.
Case Management:
Monitor client progress and adjust support strategies as needed. Connect clients with community resources, and support networks tailored to their unique circumstances. Document patient interactions and notes in our electronic health record (EHR) system Experience Demonstrated experience working with individuals with mental health conditions, substance use disorders or outpatient clinic environments. Proficiency in electronic health records (EHR/EHR management) systems and familiarity with HIPAA compliance standards. Excellent communication skills for client interaction and working effectively with families from diverse cultural backgrounds.
Pay:
$28.00 - $35.00 per hour
