Essential Duties and Responsibilities Client Care Provide direct supervision and support to residents in accordance with individualized service plans. Monitor residents' physical, emotional, and behavioral status. Conduct regular wellness and safety checks. Encourage healthy routines, personal hygiene, nutrition, and medication compliance (within scope of practice). Assist with transportation to appointments, and community activities when assigned. Behavioral Health Support Implement behavioral interventions as directed by the clinical team. Promote recovery-oriented, trauma-informed, and person-centered care. Observe and report changes in behavior, mood, or mental status. Support crisis intervention efforts following agency policies. Maintain professional boundaries while building therapeutic relationships. Documentation Complete shift notes before the end of each shift. Document incidents, observations, and interventions accurately and objectively. Complete required forms, logs, medication observation records (if applicable), and communication reports. Maintain confidentiality in accordance with HIPAA regulations. Group Activities Facilitate or assist with psychoeducational groups, recreational activities, and life skills training. Encourage participation in recovery groups, wellness activities, and community outings. Support clients in achieving individualized treatment goals. Safety and Environment Maintain a clean, organized, and safe living environment. Perform environmental safety inspections.
