Starting Pay:
$18.50
Hours:
Monday-Friday 8am-5pm Bonuses, PTO, 401k, and more.
Location:
San Tan Valley, Maricopa City & Queen Creek The Role:
Care Mentor Care Mentors fill a critical gap in integrated care. You will provide non-clinical, peer-oriented support that empowers members to navigate challenges, increase stability, and strengthen their connection to their care team. You are the "boots on the ground" ensuring no member falls through the cracks.
A Typical Day:
Your day begins from your home as you travel to meet members in their settings.
Member Visits:
See 5 members per day (approx. 1 hour each) to address goals, barriers, and recovery skills.
Clinical Support:
Complete vital signs, medication reconciliation, and depression screenings.
Coordination:
Use your 30-minute blocks between appointments to coordinate healthcare, find social resources (SDOH), and ensure members are maximizing their AHCCCS benefits.
Collaboration:
Work daily with Behavioral Health Specialists (BHS), NPs, and Care Coordinators.
Responsibilities:
Build trusting, therapeutic rapport using strengths-based communication. Provide behavioral health support (coping skills, grounding strategies, and motivational check-ins). Identify social determinants of health (housing, food, transport) and elevate concerns to the care team. Conduct outreach via phone or home visits to re-engage members. Document all interactions accurately in accordance with HIPAA and AHCCCS standards.
Requirements & Skills:
Education:
High School Diploma or equivalent required.
Certification:
Certificate as a Peer Support Specialist, Behavioral Health Tech (BHT), Medical Assistant (MA), CAN, Case Manager or equivalent is highly recommended.
Experience:
Proven experience in peer support, behavioral health, or patient management.
Skills:
Excellent time management, social perceptiveness, and the ability to work independently in the field.
Compliance:
