In this role, you'll provide direct behavioral support, assist with daily living skills, and help implement care and behavior plans created by clinical professionals. You'll promote safety, dignity, and person-centered care while documenting progress and changes in behavior. Responsibilities Provide direct behavioral support and supervision to individuals in home and community settings Assist with daily living skills, including hygiene, meals, and routines Implement behavior support and skill-building plans as directed by clinical staff Observe, document, and report changes in behavior, mood, and safety concerns Support community integration by accompanying individuals to appointments and activities Promote a person-centered, respectful, and empowering environment Follow crisis prevention and de-escalation techniques to maintain safety Collaborate with interdisciplinary team members and families to support care plans Maintain accurate, timely documentation in accordance with Sevita policies Participate in ongoing training and professional development opportunities Required Skills Behavioral health support Crisis de-escalation techniques Observation and documentation Medication reminders (per policy) ADL (activities of daily living) assistance Behavioral data collection Person-centered care planning support Safety and risk awareness Basic computer and EMR usage Transportation of individuals/community integration
