Contract Paramedic (clinical support) Reports to: Supervising Dentist and Qualified Anesthesia Provider (dentist with GA permit, physician anesthesiologist, or CRNA)

Ohio dental surgery facility (office-based)Position PurposeTo support safe, compliant delivery of office-based deep sedation/general anesthesia for Pediatric/Special Heath Care Needs patients dental surgery by performing monitoring, documentation, equipment readiness, and emergency response duties that are permitted by: (1) the Ohio State Dental Board anesthesia rules; (2) the EMS scope of practice approved by the State Board of Emergency Medical, Fire, and Transportation Services (EMFTS Board); and (3) written authorization and protocols of the EMS medical director, when applicable. This role does not function as the anesthesia provider and does not independently administer or titrate anesthetic agents for planned deep sedation or general anesthesia. Scope, Supervision, and Role Delineation This position functions as a dental auxiliary team member for tasks that a dentist may lawfully delegate in Ohio and as an EMS professional only when acting under a physician medical director's written authorization and protocols. The stricter rule governs when authorities overlap.

Direct Supervision:

• During deep sedation/general anesthesia: The anesthesia provider remains present in the anesthetizing location. The paramedic's monitoring duties are solely dedicated to patient monitoring/documentation and assisting in an emergency as directed.
• Post-anesthetic monitoring: Once the patient regains consciousness and responds appropriately, recovery is monitored by an individual experienced in patient monitoring/documentation and BLS-HCP certified; the anesthesia provider remains on the premises until discharge criteria are met.

Pre-Op & Room Readiness:

Daily anesthesia machine safety checks; verify function of suction, oxygen supply, capnography, pulse oximetry, ECG, NIBP; check emergency cart (defibrillator, airway adjuncts, emergency drugs) and document per policy.

• Patient Preparation (within delegation limits): Obtain/record baseline vitals; apply monitors (ECG, NIBP, SpO₂, capnography sampling); assist with positioning, preoxygenation, and IV setup. Initiate peripheral IV access only if authorized under EMS medical director protocols; otherwise assist the licensed provider.

Intraoperative Monitoring:

Continuously observe and document vitals at required intervals; maintain time-oriented anesthesia record; promptly report trends/alarms; keep airway/resuscitation equipment immediately available.

Medication Handling:

Prepare and label syringes and IV fluids as directed by the anesthesia provider. Do not independently decide, administer, or titrate anesthetic/sedative agents used for planned deep sedation/general anesthesia.

Airway & Emergencies:

Assist with airway maneuvers and equipment as directed. Perform advanced airway or medication interventions only during an acute emergency and only if authorized within EMS protocols and training.

Recovery Phase Monitoring:

When the patient has regained consciousness and responds appropriately, provide continuous monitoring and documentation until discharge criteria are met; ensure timely handoff and discharge instructions.

Emergency Preparedness:

Participate in and document required emergency drills (at least quarterly), including pediatric scenarios; maintain readiness for malignant hyperthermia, laryngospasm/bronchospasm, anaphylaxis, hypoxia, hypotension, arrhythmias, and cardiac arrest.

Documentation & Quality Assurance:

Complete anesthesia and recovery records, equipment checks, drug logs, adverse event reports; participate in chart audits and quality improvement meetings.

Infection Control & Safety:

Follow facility policies and

OSHA/CDC

best practices for PPE, sharps safety, and equipment reprocessing; maintain clean, safe work areas.

Team Communication & Family Support:

Support child

• and family-centered care; communicate clearly and professionally with the clinical team and guardians. Prohibited Tasks
• Acting as the general anesthesia provider or moderate-sedation provider.
• Independently administering or titrating anesthetic/sedative agents (e.g., propofol, ketamine, volatile agents, ultra-short-acting barbiturates) for planned dental anesthesia.
• Functioning in any role other than monitoring during deep sedation/general anesthesia (e.g., circulating, assisting with the dental procedure) while assigned as the monitor.
• Performing dental diagnosis, treatment planning, or other non-delegable dental tasks; performing tasks outside EMS and Dental Board rules.
• Providing EMS-level services without written authorization/protocols from an Ohio EMS medical director; exceeding EMFTS Board-approved scope under any circumstances. Qualifications
• Current Ohio Paramedic (EMT-P) certificate in good standing.
• Current BLS for Healthcare Providers (AHA or equivalent)

• Experience in patient monitoring and medical documentation in procedural or critical-care settings.
• Competency with capnography, pulse oximetry, ECG monitoring, and NIBP devices.
• Successful completion of facility orientation, emergency drills, and annual competencies; adherence to EMS medical director protocols where applicable.
• Ability to lift/move patients and equipment per facility policy; effective communication and teamwork skills; adherence to HIPAA/confidentiality.

Physical Demands / Schedule Standing for long periods; pushing/pulling equipment; assisting with patient transfers; responding rapidly to emergencies.

Standard shift:

8 hours (or as scheduled by the facility); occasional early starts based on case load; participation in drills and mandatory training. Compliance & Training Commitments

• Maintain current certifications (BLS-HCP, ACLS, and PALS if applicable).
• Participate in quarterly emergency drills and maintain documentation as required by Ohio Admin. Code 4715-5-05.
• Maintain familiarity with facility anesthesia policies, discharge criteria, and adverse event reporting process.
• Operate strictly within Ohio EMS scope and Dental Board rules; seek clarification when in doubt and escalate to supervising provider.
