Job Summary Sleep Check, LLC is seeking a compassionate Nurse Practitioner (CRNP) to provide evaluation and management services for patients with suspected or diagnosed sleep disorders, with an emphasis on obstructive sleep apnea and related conditions. The CRNP will conduct patient office visits, obtain comprehensive medical and sleep histories, perform clinical assessments, review sleep study results and other diagnostic information, develop individualized treatment plans, order appropriate sleep related equipment and supplies, and provide ongoing patient management and education. The CRNP will play an important role in helping patients understand the health risks associated with untreated sleep apnea and the importance of diagnosis, treatment and long term compliance. Responsibilities Conduct comprehensive initial and follow-up office visits for patients being evaluated for sleep disorders. Evaluate patients for suspected obstructive sleep apnea and other sleep-related breathing disorders. Review and explain home sleep study and sleep study results to patients. Discuss the potential short-term and long-term health consequences of untreated sleep apnea, including its potential relationship to cardiovascular disease, hypertension, metabolic conditions, daytime fatigue, impaired concentration, and other health concerns. Educate patients regarding available treatment options and the importance of appropriate treatment. Monitor treatment effectiveness and patient adherence. Assess barriers to treatment compliance and work with patients to develop practical solutions. Evaluate patient risk factor conditions that may contribute to or be affected by sleep disorders. Document all patient encounters, assessments, diagnoses, treatment recommendations, and follow-up plans accurately and timely. Establish appropriate follow-up schedules and monitor patient progress.
