Position Overview:

We are seeking a Medicare-credentialed Nurse Practitioner (NP) with to join our team. This role involves providing mobile, advanced wound care treatments in various locations in Louisville ,KY and surrounding areas , ensuring the highest level of patient care.

Key Responsibilities:

Provide specialized wound care treatments in patients' homes, SNFs, and other facilities . Assess, diagnose, and develop customized treatment plans for chronic wounds . Perform advanced regenerative wound care (collagen, amniotic allographs & biologics). Follow Elemental Living's established wound care protocols and best practices to ensure consistency and high-quality patient care Educate patients, caregivers, and facility staff on wound management and prevention. Maintain accurate patient documentation and adhere to Medicare guidelines . Collaborate with healthcare teams to optimize patient care and healing outcomes.

Qualifications:
