Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is a full-time APRN or Physician Assistant who independently manages patient care in long-term care settings. Responsibilities include conducting physical exams, diagnosing and treating illnesses, developing care plans, and providing preventive services. They work autonomously, making clinical decisions within scope, and collaborate with facility leadership and interdisciplinary teams to ensure holistic, high-quality care. Key duties involve maintaining compliance, documentation, participating in quality initiatives, and promoting health and safety protocols. Qualifications require a relevant degree, at least 2 years of experience (long-term care preferred), and active APRN or PA certification.
