Overview We are seeking a dynamic and compassionate Nurse Practitioner specializing Internal Medicine to join our dedicated healthcare team. Duties Conduct detailed physical examinations and comprehensive patient assessments to determine diagnoses and develop personalized treatment plans. Prescribe medications in accordance with DEA regulations while ensuring proper documentation within EMR/EHR systems such as Point Click Care. Manage acute and chronic conditions within the post-acute and long-term care environments. Collaborate closely with physicians, nurses, therapists, case managers, and other healthcare professionals to coordinate patient care effectively. Engage in diagnostic ordering and evaluation using laboratory testing, medical imaging, sonography, and other advanced tools to inform clinical decisions. Educate patients and families on health coaching topics such as medication management, nutrition, hospice & palliative care options, and disease prevention strategies. Ensure compliance with HIPAA regulations by maintaining accurate medical records and documentation of all clinical encounters. Participate in quality improvement initiatives including utilization review, discharge planning, and infection control protocols. Experience Proven experience working as an Advanced Practice Provider (APP), specifically as a Nurse Practitioner or Physician Assistant in internal medicine or related fields such as critical care or emergency medicine. Demonstrated proficiency with EMR/EHR systems like PCC, Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation and data management. Hands-on experience in skilled nursing facilities (SNFs), assisted living communities, or long-term care settings; familiarity with nursing home operations is highly desirable. Strong background in managing complex cases involving geriatrics (including dementia and Alzheimer's care), pediatrics (including toddler and neonatal ventilator management), or post-acute care environments. Knowledge of advanced procedures including gastrostomy tube feeding management, ventilator support, cardiac catheterization preparation, or spinal tap techniques is advantageous. Experience working within hospital environments such as Level I & II trauma centers or ICU units enhances your ability to handle acute pain management and critical & intensive care scenarios. coding ensures accurate documentation of diagnoses and procedures.
