PM Location:
Apple Valley, California We have partnered closely with a prominent regional medical group to secure a full-time, dedicated clinician for their primary care services division. We are currently seeking a dedicated, patient-centered Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join an established primary care team in Apple Valley, California. Under the direction of the Medical Director, you will deliver comprehensive, high-quality primary care, manage designated patient panels, and coordinate holistic treatment plans to ensure excellent continuity of care. This position is ideal for a collaborative clinician who thrives on quality clinical care, exceptional customer service, and strong multidisciplinary communication.
Responsibilities:
Perform comprehensive physical examinations, obtain detailed medical histories, establish accurate diagnoses, and manage evidence-based treatment plans for assigned primary care patients. Order diagnostic tests, interpret clinical results, and refer patients to specialized healthcare services to support continuity of care; collaborate closely with case managers as needed. Maintain high clinical practice standards, understand Hierarchical Condition Category (HCC) coding requirements, and participate in organizational quality/performance improvement initiatives. Lead sensitive end-of-life and hospice discussions with patients and family members when clinically appropriate. Develop positive working relationships across clinical departments, participate actively in provider and staff meetings, and support the organization's overarching mission and values.
Qualifications:
Education:
Master of Science in Nursing (MSN), Doctorate of Nursing Practice (DNP), or successful completion of an ARC-PA accredited Physician Assistant (PA) program. Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license to practice medicine in the State of California. Active, unrestricted Drug Enforcement Administration (DEA) registration.
Experience:
Minimum of 2 years of dedicated practice as an Advanced Practice Provider (NP or PA) within a Primary Care or Outpatient Family Medicine setting is required. Comprehensive knowledge of primary care, preventative health, and managed care concepts. Strong critical thinking, analytical problem-solving, and clinical prioritization skills. Excellent communication and active listening skills to build strong rapport with patients, families, and clinic staff.
