We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) for a temporary assignment of approximately 6-8 months to provide pain management and musculoskeletal care to patients in skilled nursing facilities (SNFs) and/or clinic. This is a field-based position involving approximately 2-3 SNF visits per week at facilities located in and around Pasadena, North Hollywood, San Gabriel Valley, or Highland Park, . There is an option to add in an in-clinic setting at our Burbank location if needed. The ideal candidate is comfortable working independently in a skilled nursing environment, has strong clinical judgment, and has at least 1 year of experience in pain management, orthopedics, sports medicine, or a closely related specialty . This position offers the opportunity to work alongside experienced pain management physicians while providing meaningful care to patients who may have complex medical and musculoskeletal conditions.

Compensation:
starting at $75/hour, depending on experience and qualifications Responsibilities Conduct comprehensive evaluations of patients in skilled nursing facilities. Obtain appropriate pain and musculoskeletal histories and perform focused physical examinations. Evaluate, diagnose, and manage acute and chronic pain and musculoskeletal conditions. Develop and implement individualized treatment plans in collaboration with the supervising/collaborating physician and facility care team, as applicable. Manage medications appropriately and safely, including controlled substances when clinically indicated and legally permitted. Review and interpret relevant diagnostic studies, including imaging and laboratory results. Monitor patients' response to treatment and make appropriate adjustments to their care plans. Identify medication-related risks and consider relevant comorbidities when developing treatment plans. Communicate effectively with nursing staff, facility providers, patients, families, and the practice's physicians. Coordinate referrals and additional medical evaluation when clinically appropriate. Perform appropriate basic procedures within the provider's training, competency, and scope of practice. Maintain timely, accurate, and complete clinical documentation. Follow all practice, facility, and regulatory requirements regarding patient safety, prescribing, documentation, and infection control. Work independently while maintaining communication with the practice's physicians regarding patient care. Provide compassionate, professional, and patient-centered care. Qualifications Current California license as a Nurse Practitioner or Physician Assistant . Current national certification appropriate to the provider's profession. Minimum of 1 year of clinical experience in pain management, orthopedics, sports medicine, or a closely related specialty. Experience evaluating and managing acute and chronic pain and musculoskeletal conditions. Experience performing appropriate pain and musculoskeletal history and physical examinations. Experience working independently and exercising sound clinical judgment. Strong communication and interpersonal skills. Ability to effectively communicate with SNF nursing staff and other healthcare professionals. Ability to travel between assigned skilled nursing facilities. Reliable transportation and ability to travel to multiple facilities as needed. Current DEA registration, if required for the position and prescribing responsibilities. Familiarity with safe prescribing practices for controlled substances and appropriate consideration of patient comorbidities, including renal, hepatic, psychiatric, gastrointestinal, cardiac, respiratory, and sleep-related conditions. Familiarity with appropriate diagnostic evaluation for pain and musculoskeletal conditions, including imaging, laboratory testing, and urine drug testing. Experience with trigger point injections and/or basic joint injections is preferred but not required. Prior SNF, inpatient, pain management, orthopedic, sports medicine, PM&R, or similar experience is preferred. Ability to work collaboratively with physicians while functioning independently in the SNF setting. Preferred Qualifications The following experience is a plus: Prior experience providing care in skilled nursing facilities. Pain management experience. PM&R, ortho, or sports med experience. Experience managing medically complex patients. Experience with controlled-substance prescribing and medication monitoring. Familiarity with interventional pain procedures and appropriate indications/contraindications. Familiarity with urine drug screening and medication compliance monitoring. Compensation The expected pay range for this position starts at $75 per hour , depending on experience and qualifications. This is a temporary per diem position for an anticipated 6-8 month assignment.
