Community Health / Primary Care We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join a collaborative primary care team in Chowchilla, CA. This is an opportunity to provide comprehensive family and preventive care to a diverse community in a team-based clinical environment. Responsibilities Provide comprehensive primary and preventive care Evaluate, diagnose, and treat acute and chronic conditions Develop treatment plans and coordinate referrals and follow-up care Perform appropriate office-based procedures Maintain accurate and timely clinical documentation Collaborate with physicians and other healthcare professionals Support quality improvement and community health initiatives Requirements Graduate of an accredited NP or PA program Active California NP or PA license, or eligibility 12+ months of clinical experience preferred Primary care/family practice experience preferred Strong communication and patient-care skills Compensation & Benefits $150,000-$212,000 annually Full-time permanent position Comprehensive benefits package Collaborative team-based environment Professional growth and development opportunities to learn more about this Family Practice opportunity in Chowchilla, CA.
