Job Description
Overview Community Health Partners is seeking a full‑time Advanced Practice Provider (APPs) to join the Perioperative Assessment Clinic team. In this role, the APP will provide comprehensive perioperative care for elective surgeries and endoscopic procedures by collaborating with an interdisciplinary care team. Responsibilities include conducting detailed clinical assessments, developing and managing treatment plans, performing medical diagnoses and prescribing appropriate therapies and treatment plans. The APP will support patients throughout the surgical continuum, including pre-operative assessment and optimization as well as perioperative and postoperative care recommendations, ensuring the highest standards of safety and quality. - Responsibilities In this role, you will: Works with the Clinic Medical Director and multidisciplinary team to develop, implement, evaluate, and refine evidence-based systems of care for preoperative evaluation and optimization.
Qualifications Education and Experience Minimum Requirements:
Completion of a Nurse Practitioner or Physician Assistant program required Credentialed by the CMC Medical Staff Office as anAllied Health Professional Licenses and Certifications:
If qualifying as a Nurse Practitioner, the following is required NP - Current State of California Nurse Practitioner License required NPF - Nurse Practitioner Furnishing Number required Experience Three of more years of experiences preferred in a perioperative area, time as a registered nurse counts Licenses and Certifications- Eligible to be licensed to practice Medicine in the state of California required Disclaimers
