This is purpose-driven work, on wheels. Find your way back to the joy of taking care of your community. General Tasks As a Family Nurse Practitioner, you will:Primary Care Services:
Conduct physical exams, diagnose and treat acute and chronic illnesses, and provide routine primary care to improve patient health outcomes.Patient Assessments:
Take medical histories, perform physical examinations, and order and interpret diagnostic tests.Chronic Disease Management:
Develop and manage treatment plans for chronic conditions such as diabetes, hypertension, and asthma.Documentation:
Maintain accurate and timely patient records using electronic health record (EHR) systems.Administrative Duties:
Including but not limited to assisting with opening/closing the clinic, inventory, patient follow-up, verifying insurance, collecting payment, scheduling appointments, and maintaining a clean environment.Preventive Care:
Administer vaccinations, conduct health screenings, and educate patients on disease prevention and healthy lifestyles.Collaboration:
Work with other healthcare providers, nurses, and medical assistants to deliver comprehensive care.Community Outreach:
Participate in outreach activities to raise awareness about the mobile clinic's services and engage with the community. Actively participating in leadership-level planning for the best growth opportunities.Qualifications & Requirements Values :

Master's or DNP Family Nurse Practitioner (FNP) with completion of an accredited FNP program, with current Family Nurse Practitioner (ANCC or AANPCB) Board Certification or equivalent certification. Currently licensed or eligible for California APRN licensure as aFamily Nurse Practitioner Experience:
Clinical experience in family and primary care services preferred Experience with EHR preferred Bi-lingual preferredPerformance :

Work Environment 100% Onsite Travel to various community locations, often in underserved or rural areas Flexible working hours, including some evenings or weekends, depending on the clinic's schedule Regular work on and around the mobile unit Community-facing role requiring strong interpersonal communication and professionalism Fast‑paced environment with outdoor work and varied weather conditions When mobile clinic events are not scheduled, this position will operate as a Family Nurse Practitioner within the brick-and-mortar clinic as needed Salary & Benefits 80 PTO hours available after the first 60 days of service Medical insurance- 100% employer contribution, participating plans $10K Life insurance
