Part-Time (Tuesday-Thursday) Join a Growing Nephrology Practice We are a well-established, physician-led practice seeking a compassionate, motivated Nurse Practitioner to provide high-quality medical care to patients in skilled nursing facilities throughout Orange County and Los Angeles. This is an excellent opportunity for an NP who enjoys building long-term patient relationships, practicing independently while collaborating closely with physicians, and maintaining a predictable weekday schedule. Responsibilities Provide comprehensive medical evaluations and follow-up care in skilled nursing facilities. Manage acute and chronic medical conditions in collaboration with supervising internists. Develop and implement treatment plans. Order and interpret diagnostic tests and laboratory results. Coordinate care with facility staff, specialists, and families. Complete accurate and timely documentation within the EMR. Participate in interdisciplinary care planning. Qualifications Current California Nurse Practitioner license. National NP certification. DEA license (or eligibility). Excellent clinical judgment and communication skills. Ability to work independently while collaborating with physicians. Reliable transportation for travel between facilities. Experience in internal medicine, geriatrics, family medicine, hospital medicine, or post-acute care preferred, but motivated new graduates are encouraged to apply.
