Our providers deliver comprehensive medical care directly within assisted living, memory care, and senior living communities. Our model is centered on accessibility, continuity, clinical quality, and responsiveness. By caring for patients where they live, our providers develop meaningful longitudinal relationships with residents while collaborating closely with the families, community teams, and healthcare organizations involved in their care. We are seeking experienced and compassionate Nurse Practitioners and Physician Assistants to join our growing mobile provider team throughout Orange County and LA County. Position Overview The Mobile Primary Care Provider is responsible for delivering comprehensive primary care to older adults directly within senior living communities. This is an autonomous, field-based clinical position requiring strong assessment skills, sound medical judgment, excellent communication, and the ability to manage a medically complex geriatric patient population. Providers manage both acute and chronic medical conditions, perform comprehensive evaluations, develop treatment plans, monitor changes in condition, coordinate care across the healthcare continuum, and establish strong working relationships with senior living teams and families. The successful provider is clinically confident, organized, responsive, and comfortable taking meaningful ownership of their patient panel. Key Responsibilities Perform comprehensive patient histories, physical examinations, assessments, and follow-up visits Diagnose and manage acute and chronic medical conditions Develop individualized treatment plans based on patient needs, goals, clinical findings, and relevant medical history Provide ongoing management of common chronic conditions affecting older adults Provide preventive care and age-appropriate health maintenance Evaluate changes in condition and determine appropriate clinical interventions Order and interpret laboratory testing, diagnostic imaging, and other appropriate studies Prescribe medications and other treatments within applicable scope of practice Perform medication reconciliation and ongoing medication management Review hospital, specialist, laboratory, imaging, and other relevant clinical records Geriatric and Longitudinal Care Manage medically complex older adults with multiple chronic conditions Maintain awareness of each patient's active medical problems, treatment plans, and outstanding clinical needs Evaluate functional, cognitive, behavioral, and social factors affecting patient health when clinically appropriate Identify changes in condition early and develop appropriate treatment or escalation plans Monitor patients longitudinally and adjust treatment based on changes in health status Consider patient goals, quality of life, family involvement, and care setting when developing treatment recommendations Collaborate with hospice, home health, therapy, behavioral health, and other supportive services as clinically appropriate Care Coordination and Transitions Coordinate care with specialists, hospitals, skilled nursing facilities, rehabilitation facilities, pharmacies, home health agencies, hospice organizations, therapy providers, and diagnostic services Review discharge information following hospitalizations, emergency department visits, rehabilitation stays, and other transitions of care Develop appropriate post-discharge treatment and follow-up plans Communicate provider recommendations clearly to patients, families, senior living staff, Case Managers, and other members of the care team Work closely with Optima Case Managers to address outstanding referrals, testing, barriers, and other patient needs Identify patients at elevated risk for hospitalization, emergency department utilization, or clinical deterioration Escalate or transfer patients to a higher level of care when clinically appropriate Senior Living Community Partnership Develop strong clinical relationships with executive directors, wellness directors, nurses, medication technicians, caregivers, and other senior living staff Maintain responsive communication regarding patient concerns and changes in condition Serve as a trusted clinical partner within assigned communities Participate in care conferences and interdisciplinary discussions when appropriate Educate community staff and families regarding treatment plans and patient needs within appropriate clinical and privacy boundaries Maintain a professional, reliable, and solution-oriented presence within partner communities Represent Optima Health with clinical excellence, compassion, accountability, and professionalism Documentation and Clinical Accountability Complete accurate and timely clinical documentation within the electronic health record Maintain complete problem lists, medication lists, assessments, and treatment plans Document clinical reasoning, patient status, orders, and follow-up plans appropriately Review and address laboratory results, diagnostic testing, messages, and other clinical information within established timeframes Maintain compliance with applicable scope-of-practice requirements, HIPAA, Optima policies, and healthcare regulations Participate in quality improvement, clinical review, and provider meetings as appropriate Maintain accountability for unresolved clinical needs within the assigned patient panel Qualifications Active and unrestricted California Nurse Practitioner or Physician Assistant license Applicable national board certification DEA registration as required for the position Prescriptive authority consistent with applicable California requirements Strong clinical assessment, diagnostic, and medical decision-making skills Ability to independently manage acute and chronic medical conditions Excellent written and verbal communication skills Strong organizational and time-management skills Comfortable working autonomously in a mobile clinical environment Ability to manage a patient panel and multiple competing clinical priorities Strong documentation skills and comfort using electronic health records Commitment to responsive communication and clinical follow-through Valid driver's license, reliable transportation, and ability to travel regularly throughout the assigned territory Preferred 1-2+ years of experience as a Nurse Practitioner or Physician Assistant Experience in geriatrics, primary care, internal medicine, family medicine, skilled nursing, home-based primary care, or senior care Experience managing medically complex older adults Experience working within assisted living, memory care, or senior living environments Experience with transitions of care and post-hospital management Familiarity with Medicare, Medicare Advantage, or value-based care Experience collaborating with home health, hospice, therapy, specialists, and other healthcare partners Bilingual English/Spanish What Success Looks Like
