full-time A growing pain management practice is hiring NPs and PAs to provide specialized pain and palliative care to geriatric residents across skilled nursing facilities in Richmond and the East Bay. Geriatric pain in SNFs is chronically undertreated - you'd be the provider who actually assesses and manages it, following the same residents over time rather than cycling through a rotating panel. New graduates welcome with structured onboarding; 5+ years of relevant RN experience also counts. Malpractice with tail coverage included. Practice actively adding facilities - room to grow. Key Responsibilities Conduct thorough pain assessments including medical history, physical exam, and identifying source and severity Build individualized, evidence-based treatment plans combining medication, non-drug approaches, and interventional options Manage prescriptions responsibly per state and federal regulations Coordinate referrals for physical therapy, acupuncture, spinal blocks, and interventional care Guide patients and families through pain management strategies and lifestyle modifications Partner with physicians, physical therapists, and facility providers to keep care aligned Track treatment effectiveness through regular follow-ups and adjust plans as needed Maintain accurate, current documentation in TEBRA and PointClickCare Build lasting relationships with leadership and staff at each partner facility

Qualifications Required:

Active, unrestricted California NP or PA license AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification Fully onsite; comfortable traveling between East Bay facilities New graduates welcome; 5+ years relevant RN experience also counts Strong communication, empathy, and sound clinical judgment

Preferred:

Experience with geriatric patients Background in skilled nursing, long-term care, palliative care, or hospice Solid grasp of pain pharmacology and psychosocial aspects of chronic pain TEBRA or PointClickCare familiarity Active Medicare credentialing strongly preferred Schedule & Compensation $60-$100/hour + productivity bonus | ~$115,200-$192,000/year Full-time | Schedule varies by facility and caseload Onsite at Richmond and East Bay skilled nursing facilities Malpractice with tail coverage included Structured onboarding program
