Job Description

Job Overview We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) or PA to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive primary care services across diverse patient populations, including pediatrics, geriatrics, and adult patients. Your expertise will help promote health, prevent illness, and manage acute and chronic conditions. This position offers an exciting opportunity to work in a good environment that emphasizes patient-centered care, utilizing advanced clinical skills and a broad scope of practice to make a meaningful difference in patients' lives. We are opening a new community outpatient clinic in Rocklin, CA, to provide accessible adult primary and general medical care. This is a small, physician-supported practice—not a hospital or procedure-based specialty setting. You will work with experienced physicians and staff, including clinicians trained at world-renowned institutions such as the Mayo Clinic, and alongside medical and nursing students in a teaching-oriented clinic. The PA/NP will help shape clinic workflows, the patient experience, and a sustainable panel from the ground up, with close collaboration from our supervising physician(s). We are seeking a Physician Assistant to care for adult general medicine patients.
This is a clinic-based role:
office visits, chronic disease management, acute complaints, follow-up care, and coordination of treatment with the clinic team. There is no hospital, ICU, emergency department, or inpatient coverage. Responsibility/Duties Hypertension, diabetes, and lipid disorders, Thyroid disease, COPD and asthma, Common infections, Musculoskeletal complaints, Preventive care, Medication management You will practice under a collaborative agreement with our supervising physician. Communicate clearly with patients, families, and the clinic team See scheduled and same-day outpatient visits for adult general medicine patients Perform histories and physical exams; order and interpret laboratory tests and outpatient imaging Diagnose and manage common acute and chronic conditions Prescribe medications in accordance with California PA/NP law and our collaborative practice agreement Document visits accurately in the EMR, Educate patients on treatment plans, medications, and preventive care Coordinate referrals, results follow-up, and prior authorizations as needed Perform office procedures appropriate to general medicine—such as simple suturing, incision and drainage, and joint or trigger-point injections—if privileged and trained. On-the-job training will be available for additional clinic procedures, including Pap smears. Communicate clearly with patients, families, and the clinic team Follow HIPAA, infection-control, and clinic protocols Assist with follow-up care. Qualifications Knowledge of CPT coding, ICD coding (ICD-10/ICD-9), and healthcare compliance standards such as HIPAA. Excellent communication skills for triage assessments, patient education, interdisciplinary collaboration, and documentation accuracy. Demonstrated ability to work effectively in outpatient clinics or post-acute care environments; telehealth experience is beneficial. Join us in delivering exceptional healthcare that empowers patients through compassionate service and innovative practices Requirements Licensed FNP or PA in California National certification (AANP/ANCC or NCCPA) BLS (ACLS preferred) DEA, or ability to obtain Comfort managing adult primary care in an outpatient setting Strong documentation and communication skills Knowledge of ICD-10 and CPT helpful License, DEA, and dues support Preferred One or more years of outpatient internal medicine, family medicine, or similar clinic experience (new graduates with strong primary-care rotations are welcome to apply) Experience with chronic disease management and Medicare/commercial documentation Schedule and benefits Monday-Friday, 10:00 a.m.-6:00 p.m. Malpractice coverage CME allowance and paid CME time License, DEA, and dues support To apply, submit your resume and a short note about your outpatient or primary care experience.
