2994255267-2 Apply for this job now Job Description Great opportunity in Sacramento, CA for a Nurse Practitioner seeing Post Transplant Care patients working for a private group. Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) Minimum 2 yrs exp as NP preferably with transplant, nephrology, or complex chronic disease populations 40 hours a week

• Mon-Fri 8a-5p Duties will include working in a clinic 3 days a week and 2 days a week doing comprehensive health work via telehealth for lab review and follow-ups Rare visits to home
• maybe once a month if that would be required when necessary Clinic flow
• patients get labs drawn in clinic, then see NP, then NP reviews labs 48 hours later, and then follows up with patients and nephrologists Program aims to proactively manage care
