What You'll Do Deliver high-quality, compassionate care to patients in their homes and at various facilities. Use comprehensive medical records and a dedicated support team to build and carry out personalized treatment plans. Communicate clearly and empathetically with patients, families, and the care team explaining disease processes and treatment options with patience. Care for patients holistically, honoring their wishes and involving their families in decisions. What We're Looking For Licensed Nurse Practitioner with at least two years in primary care (more is a plus!). Excellent bedside manner, strong primary-care knowledge, and a warm, personable approach. A natural ability to build trust with patients and families through clear explanations and compassionate care. A responsive, reliable team player with a strong work ethic. Someone motivated to keep improving, both clinically and professionally. Palliative care experience is a plus! Schedule & Flexibility Flexible scheduling within day time hours. No formal on-call requirement though you should be ready to address the occasional urgent message about your patients.
