$60.00-$70.00 per hour About the Role LaVida Heal PLLC is a growing Colorado wound care group providing evidence-based care across skilled nursing facilities, long-term care, and outpatient settings. We are looking for a dedicated NP or PA to join our team and help deliver high-quality wound care to patients throughout Colorado. Responsibilities Evaluate and manage complex wounds, including pressure injuries, diabetic ulcers, surgical wounds, and chronic wounds. Provide wound care in SNF, LTC, and/or outpatient settings. Document patient visits and collaborate with facility staff, physicians, and clinical leadership. Use evidence-based protocols and advanced wound care modalities. Exercise independent clinical judgment while working with the support of a medical director and lead NPs. Qualifications Active Colorado NP or PA license. Wound care certification (CWOCN, CWS, or WCC) preferred, or willingness to obtain certification. SNF, LTC, or wound care experience preferred. Valid driver's license and reliable transportation. Strong communication skills and the ability to work independently.
